Our office is committed to providing quality and cost-effective healthcare to our patients. Insurance reimbursement is a contract between you and your insurance company.
It is your responsibility to understand the limits and restrictions for your insurance policy. As a courtesy to you, we will file a claim with your primary insurance for services rendered.
We require a current copy of your insurance card and we will need to be informed of any change in insurance status at every office visit. You will be responsible for all co-pays, deductibles, and co-insurance amounts not covered by your primary insurance. We do not bill secondary insurance.
