To begin, I would like to say THANK YOU for the trust you have given me over the years as your physician. I've known some of you for more than 15 years! Many of you have become dear friends of mine outside of the office. It has been an honor serving the Northern Virginia community for the past seventeen years. Your continued wellness and health are a priority to me.
I am writing to inform you that I will be permanently closing my practice effective July 31, 2021 due to relocation. A notification of impending closure was emailed to current patients in early May. Current patients are those patients that have been seen in the office within the past two years. It is important that you continue receiving exceptional medical care. You should establish contact with another physician as soon as possible for your ongoing gynecological care. You may continue with any Board-Certified OB/GYN provider that participates with your health plan. You can also check to see if your general practitioner performs routine well-woman exams. Please click HERE to search for providers in your area. You will be able to see if they are accepting new patients.
We would be happy to send a copy of your medical records to your new physician. Please do not submit your request until you have secured an appointment date and time. To initiate the request simply visit our website and print the Release of Medical Records form. The completed form can be mailed or submitted electronically. Upon receipt, we will forward a copy of your medical records to your designated provider. Rest assured that our office will remain available during the upcoming months to consult with your new provider as necessary. It is important to me that you have a smooth transition of care. In accordance with the Virginia Board of Medicine, your records will be maintained for a minimum of six years following your last encounter at our office.
There will be limited Telemedicine appointments available in July. Appointments are filling quickly. Appointments can be scheduled using our convenient online patient scheduling feature. Click the Patient Portal tab on our website to check availability.
All inquires and signed Release of Medical Records forms should be directed to firstname.lastname@example.org. Please note that prescription refill requests will no longer be processed through our office. Contact your primary care provider if you have an urgent refill request. You can also visit Nurx.com if you need to get a refill before you can be seen by your new provider.
This has been a remarkable journey! I appreciate all of you for entrusting me with your care. I hope I've been a valued partner & resource when it comes to your health & well being. And no, I am not retiring. LOL. I'll be moving to North Carolina and joining a practice in the Raleigh/Durham area. While I would love to personally say goodbye to each & every one of you, that's simply not possible. I will instead be hosting a "Farewell Event" in late August. I hope you can attend.
